曾獲得繪畫大獎的青年木股英雄（忍成修吾 飾），卻在某天失心發狂，殺死了擔任他模特的女友富江（酒井美紀 飾）。木股的朋友秀日和青山巧（妻夫木聰 飾）目睹這一切，並幫忙搬運掩埋屍體。三人相約保守這個秘密，卻不知道更為恐怖的事情還未到來。三人在酒吧散心期間，原本應該死去的富江竟離奇地出現在包間內，木股不堪承受巨大的精神壓力，旋即於洗手間內自殺。秀日嚐試找到富江的屍體，結果卻因此變得愈加瘋狂。與此同時，富江又頻繁周旋青山身邊。這個神秘恐怖的女孩將三個男生相繼逼向崩潰的邊緣……
本片根據日本著名恐怖漫畫家伊藤潤二的同名係列漫畫改編。
