薩曼莎（凱特·瑪拉 Kate Mara 飾）是某高中校報的記者，她因報道校隊的醜聞而被列入禁止參加返校節的黑名單，她的兩位好友金娜和明迪（Olesya Rulin 飾）也一同遭殃。三個女孩無所事事，聚在一起講故事打發時光。她們先是講述了1969年鹽湖城一個名叫瑪麗的乖乖女被校隊成員殺害的傳說，隨後又提到了恐怖的“血腥瑪麗”。在此之後，三個女孩神秘消失，次日則在郊區的廢棄工廠出現。雖然當事人認定這起事件實為球隊男孩們所為，但薩曼莎卻從這一刻起經常被“血腥瑪麗”的歡迎所困擾。
不久，球隊一名男孩離奇死亡，薩曼莎相信，似乎被遺忘數十年的恐怖惡靈正在慢慢蘇醒，並展開她的血腥複仇……
